Insurance
I am a Preferred Provider with Regence Blue Cross of Oregon, Reliant, ODS Health Plan and Samaritan Health Plans. Other insurance companies will reimburse for my services if your individual plan has out of network benefits. Insurance plans vary and it is important to contact your insurance provider and clarify your benefits. If you have questions about whether your insurance will cover my services you may contact me.
Billing
Session Fees: Fees for Intake Session/Diagnostic Interviews are due in full at the time of session. All other session payments are due in full by cash or check at the time of the session unless prior arrangements are made.
